fix(SESF-74): rebuild the open last turn, replace its stale row, repair indexed duplicates - #96
transcript_parser.parse_transcript flushed the last turn whenever a transcript ended on a complete line, and moved the offset past its user line. The file watcher (2 s after each write) and the provider backfill both parse mid-turn, so assistant text written later had no user line and was dropped. When one path saw a turn mid-write and the other saw it complete, both rows stayed, because the content-hash doc_ids differ.
Measured on claude_code_cli:
2,856 duplicate (session_id, turn_index) groups. In 2,805, the user-only row came from the watcher (bare-filename source_path, no source_id).
1,497 user-only rows whose transcript on disk has the reply (619 watcher, 878 provider).
1,364 user-only rows from deleted transcripts, and 204 genuine user-only turns.
Change
parse_transcript_incremental returns ParseResult(turns, consumed_offset, resume_offset). The open last turn is emitted so it is searchable, and the next parse resumes at its user line. parse_transcript keeps its (turns, consumed) return value.
index_state stores consumed_offset next to last_byte_offset. Watcher backfill compares file size with consumed_offset. The Claude provider skips a file whose size equals its cursor's consumed_offset, so idle transcripts are not re-parsed.
Parser turns carry REPLACE_SAME_TURN (in provider_adapters). After add_turns stores a batch, _replace_stale_turns deletes other chunk_type == "turn" rows of the same session and turn_index. This also runs when the new doc_id was already indexed. Summaries and unflagged providers (Codex) are untouched. Deletes run only after the new row is stored.
cleanup.py dedupe-turns (dedupe_turns.py) repairs existing rows. It re-parses affected sessions on disk through the Claude adapter and passes their turns to add_turns. Where the transcript is gone, it keeps the longest row. Dry run by default; --apply refuses without --yes.
Trade-off
While a turn is in progress, it re-embeds each time its text changes (at most once per watcher debounce), through the same embedding budget.
Spec changes in older tests
test_upstream_ports.py: the watcher backfill tests stub parse_transcript_incremental, which the watcher calls.
test_provider_cowork_legacy.py: the cursor test checks consumed_offset > 0. Its one-turn fixture ends on an open turn, so last_byte_offset resumes at 0.
After merge
Restart the server.
cleanup.py dedupe-turns dry run: 2,855 duplicate groups and 3,067 user-only turns across 1,588 sessions.
cleanup.py dedupe-turns --apply --yes, with SESSIONFLOW_MILVUS_URI set.
Checks

pytest: 812 passed, 120 skipped. New tests in tests/test_open_turn.py and tests/test_dedupe_turns.py.
Summary by CodeRabbit
Bug Fixes
Improved transcript indexing for conversations that are still in progress, so replies can update the correct turn without leaving stale duplicates.
Improved backfill handling to detect transcript changes that were previously missed.
New Features
Added a command to identify and repair duplicate or incomplete indexed turns. It previews changes by default; applying repairs requires explicit confirmation.
parse_transcript flushed the last turn whenever a transcript ended on a
complete line and moved the offset past its user line. The watcher (2 s
after each write) and the provider backfill both parse mid-turn, so the
assistant text written later had no user line and was dropped. When one
path saw the turn mid-write and the other saw it complete, both rows
stayed, because the content-hash doc_ids differ.
- parse_transcript_incremental returns a ParseResult with consumed_offset
and resume_offset. The open last turn is emitted, and the next parse
resumes at its user line. parse_transcript keeps its (turns, consumed)
return value.
- index_state keeps consumed_offset next to last_byte_offset. Backfill
compares the file size with consumed_offset, and the Claude provider
skips a file whose size equals its cursor's consumed_offset.
- Parser turns carry REPLACE_SAME_TURN. After add_turns stores a batch,
_replace_stale_turns deletes other chunk_type 'turn' rows with the same
session_id and turn_index, also when the new doc_id was already indexed.
Summaries and unflagged providers are untouched.
- Watcher, backfill, the index hook and the Claude provider use the new call.
- test_upstream_ports stubs parse_transcript_incremental, and the cowork
cursor test checks consumed_offset: a one-turn fixture resumes at 0.
dedupe_turns.plan scans the Claude Code turn rows in Milvus for groups of
one session and turn_index that hold several rows, or one row without a
reply. apply re-parses each affected session's transcript through the
Claude adapter and passes the affected turns to add_turns, whose same-turn
replacement deletes the stale rows. A rebuilt turn keeps the project_root
of its longest indexed row. A session whose transcript is gone keeps the
longest row of each duplicate group.
Dry run by default; --apply refuses without --yes. The live dry run finds
2,855 duplicate groups and 3,067 user-only turns across 1,588 sessions.
CLAUDE.md and AGENTS.md describe the open-turn rule and the command.
Transcript ingestion now emits complete open turns and tracks separate resume and consumed offsets. Indexing replaces stale rows for flagged turns. A new dedupe-turns command plans or applies repairs to existing indexed rows.
The incremental parser returns turns with separate consumed and resume offsets. It emits complete open turns and resumes at the turn’s user line.
Ingestion and offset state file_watcher.py, http_server.py, provider_claude.py, tests/test_open_turn.py, tests/test_provider_cowork_legacy.py, tests/test_upstream_ports.py
The watcher, HTTP endpoint, and Claude adapter use incremental parse results. They store or return both offsets.
The repair command plans duplicate and user-only groups, rebuilds matching transcript turns, and applies fallback deletion when no matching turn is available. It defaults to dry-run; applying requires --yes.

The repair command can report success while some turns remain unrepaired; verify submitted turns before relying on its result. Ordinary indexing also performs avoidable client setup. These are bounded risks rather than broad ingestion failures.

Explanation
The title names three changes that all appear in the changeset: open-turn rebuilding in the transcript parser and providers, same-turn stale row replacement in rag_engine.add_turns via REPLACE_SAME_TURN, and the new cleanup.py dedupe-turns repair command with dedupe_turns.py. It is a single conventional-commit sentence and is specific enough for a teammate scanning history. It is long at 90 characters, but that does not make it misleading or vague.

A replacement whose FTS delete failed leaves a row that Milvus no longer
holds but keyword search still returns, and the Milvus-only scans could
not find it again (Codex review on #96).
- dedupe_turns._indexed_rows reads Milvus and the FTS sidecar and merges
them by doc_id.
- apply deletes every other row of a rebuilt group by doc_id after
add_turns, so delete_by_doc_id clears FTS-only copies.
The live dry run finds 2,856 duplicate groups, one more than the
Milvus-only scan.
_replace_stale_turns opens milvus_client(db_path) even when keep is empty. add_turns calls it for every batch, including Codex and other unflagged providers and summaries. Each call adds a client open and a collection check (_ensure_collection) that does nothing. In non-server mode, each call also opens a new MilvusClient. Skip the work when keep is empty.

add_turns defers turns when the embedding budget refuses a batch (paused,
or the per-run cap reached) and returns normally. dedupe_turns.apply still
deleted every superseded row, so a deferred turn lost its only indexed
copy (Codex and CodeRabbit review on #96).
- apply queries Milvus for each rebuilt doc_id after add_turns and deletes
a group's old rows only when its replacement is stored. Unstored turns
count as deferred_turns, keep their rows and stay in the next plan.
- cleanup.py dedupe-turns reports deferred rebuilds and exits 1.
- _replace_stale_turns returns before opening a Milvus client when no turn
in the batch is flagged (CodeRabbit nitpick).
