Request calls scopes and document scheduled X call tools

Summary

• Requests calls.read and calls.write on the X connector so an existing install can grant access to scheduled X calls after signing in again.
• Documents the six xmcp tools in the X MCP guide: schedule_call, list_scheduled_calls, get_scheduled_call, update_scheduled_call, cancel_scheduled_call, and respond_to_scheduled_call. Schedule, update, cancel, and RSVP only when the owner asked. Missing call tools while other X tools work means reconnect, not a new Project or App.

Test plan

• Confirm mcp.json and the README scope list include calls.read and calls.write, and that existing installs are told to sign in again.
• After reconnect, tools/list on the X connector includes schedule_call for an app that is allowed to call POST /2/calls/scheduled.
• A list (calls.read) does not schedule, update, cancel, or RSVP unless the owner asked.
